The story follows the main character as he is trying to find a cure for Yonah, his daughter, who is suffering from the fatal Black Scrawl, a curse that overtakes the body bit by bit, making painful black symbols and runes appear on the skin, until the victim eventually pass away. The player gains the ability to understand what the shades are saying, including the one possessing Kainé, though in-game the protagonist, Weiss, and Emil are still unable to. The game was meant to appeal to older players; it was intended as an action-role playing game (RPG) for an older market than Square Enix's action-RPG series Kingdom Hearts.

Kainé's character was made intersex, since the team felt it fit in with many other aspects of her gritty backstory. Few games can bring this many vocal tracks without distracting player from in game conversations/cutscenes. Each ending gives more and more perspective, and have such great weight to them, definitely making it worth playing through the second half of the game four times to see them all. Emil's character was derived from a female character named Halua, while Kainé was originally a far more feminine type who hid her violent nature.This influenced the decision to have a main character in his 30s for the international version, as well as more blood and swearing than typical in a Square Enix RPG. After clearing the first ending, you can load your save file to restart from the second half of the game. The term "Replicant" was borrowed by Yoko from the 1982 science fiction movie Blade Runner, although Yoko did not cite a particular source for Nier 's name, passing it off as a codename that persisted through development. Sometimes you will hear characters lamenting on the concept of life and death from a simple fetch quest, other times you might get to see NPCs doing things they don't normally do.
